Job Title: Case Management Worker

We are seeking a skilled and compassionate Case Management Worker to join our team at the Indian Youth Friendship Society. As a Case Management Worker, you will play a vital role in supporting the reintegration of offenders into the community.

Job Summary:

The Case Management Worker will be responsible for assessing the needs of offenders, developing and implementing individualized plans to support their reintegration, and providing ongoing support and guidance to help them achieve their goals.

Key Responsibilities:
  • Conduct assessments to identify the needs and goals of offenders
  • Develop and implement individualized plans to support reintegration
  • Provide ongoing support and guidance to offenders
  • Collaborate with community agencies and services to provide referrals and support
  • Monitor and evaluate the progress of offenders
Requirements:
  • College/CEGEP diploma or equivalent experience
  • Experience working with offenders and in community settings
  • Excellent communication and interpersonal skills
  • Ability to work in a fast-paced environment and meet deadlines
  • Valid driver's license and reliable transportation
