About the Opportunity

The Care Coordinator plays a vital role in assessing, planning, and authorizing services for clients referred to Continuing Care. This position requires strong assessment, interviewing, and negotiation skills to identify clients' goals, unmet needs, and resource requirements. The Care Coordinator works collaboratively with interprofessional teams to facilitate hospital discharge and support community-dwelling clients. This role involves authorizing services, making referrals, and linking clients with professional and volunteer resources to maximize available services and resources in the home/community.

About You

We are seeking a highly skilled and compassionate individual with a Bachelor's Degree in Nursing, Social Work, Occupational Therapy, Physiotherapy, or Nutritional Studies. The ideal candidate will have a minimum of three years' experience in a community or long-term care setting, with demonstrated knowledge and/or experience in dedicated case management or discharge planning. Strong computer skills, excellent verbal and written communication skills, and the ability to work independently and collaboratively in a busy multidisciplinary environment are essential. Knowledge of community agencies and resources, as well as competencies in other languages, are also highly valued.

Hours and Compensation

This is a permanent full-time position with a 70-hour bi-weekly schedule. Shifts may include days, evenings, weekends, and holidays. The successful candidate will be offered a competitive hourly rate of $46.75 and may be eligible for our benefits package, which includes health, dental, travel, long-term disability, and life insurance coverage, as well as a defined benefit pension plan.
