About the Opportunity

The Nova Scotia Health Authority is seeking a skilled Continuing Care Coordinator to join our team. As a key member of our healthcare team, you will be responsible for assessing, planning, and coordinating the care of clients referred to our Continuing Care program.

As a Continuing Care Coordinator, you will work closely with clients, families, caregivers, and healthcare teams to identify goals, unmet needs, and resource requirements. You will develop and implement care plans that maximize available services and resources to support clients in their homes and communities.

You will be responsible for authorizing services, making referrals, and linking clients with other professional and volunteer resources. You will also work with interprofessional teams to facilitate hospital discharge and return clients to their communities.

This is a dynamic and rewarding role that requires strong assessment, interviewing, and negotiation skills. You will be able to work independently and collaboratively in a busy multidisciplinary environment.

We are looking for a candidate with a strong background in healthcare, preferably with experience in community or long-term care settings. You will have a Bachelor's degree in a relevant field, such as nursing, social work, occupational therapy, or physiotherapy. You will also have current and ongoing registration with the applicable Nova Scotia Professional Regulatory body.

About You
  • Strong background in healthcare, preferably with experience in community or long-term care settings
  • Bachelor's degree in a relevant field, such as nursing, social work, occupational therapy, or physiotherapy
  • Current and ongoing registration with the applicable Nova Scotia Professional Regulatory body
  • Minimum three (3) years' experience with the professional regulatory body
  • Demonstrated knowledge and/or experience in dedicated case management or discharge planning
  • Competent with computer use, keyboarding, and able to conduct assessments using a laptop
  • Knowledge of community agencies and resources
  • Excellent assessment, interviewing, and negotiation skills
  • Excellent verbal and written communication skills
  • Ability to work independently and collaboratively in a busy multidisciplinary environment
  • Ability to multi-task, prioritize workload, and manage time efficiently
  • Valid driver's license and access to a reliable vehicle
  • Competencies in other languages preferred; French an asset
