Job Title: Visiting Nurse
Job Summary:
Closing the Gap Healthcare Group is seeking a skilled and compassionate Visiting Nurse to join our team. As a Visiting Nurse, you will provide high-quality care to clients in their homes, focusing on wound care management and other community-based services.
Key Responsibilities:
- Provide comprehensive wound care management to clients in their homes
- Administer IV antibiotics and other medications as prescribed
- Conduct regular assessments and develop care plans to meet client needs
- Collaborate with healthcare teams to ensure seamless care coordination
- Document client progress and maintain accurate records
- Order supplies and equipment as needed
- Develop and maintain strong relationships with clients and their families
Requirements:
- Current certification as a Registered Nurse (RN) or Registered Practical Nurse (RPN) with the CNO
- Minimum of two years' experience in community or clinical/hospital setting
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Strong problem-solving and critical thinking skills
- Current Standard First Aid & CPR/AED (Level C) certification
- Mask Fit test certification
