Key Responsibilities:
- Manage and coordinate care, evaluating health outcomes and educating clients and families to meet health goals
- Provide all levels of nursing care, from stable clients with predictable outcomes to those with acute, complex, and rapidly changing health needs
- Assign and delegate tasks to different categories of workers, ensuring safe and effective care
- Develop and implement care plans with clients and families, incorporating discharge planning as needed
- Implement nursing strategies and interventions according to established care plans
- Perform technical nursing skills at a safe level, adhering to nursing principles and VON policies and procedures
- Document nursing processes clearly and accurately, according to regulatory and VON standards
Requirements:
- Must be Licensed as a Registered Nurse in the province of practice
- Canadian Nurses Association certification in Community Health Nursing is an asset
- Clinical practice experience and experience in community health are assets
Skills and Abilities:
- Ability to observe and assess clinical issues effectively
- Strong written and verbal communication skills, with the ability to engage with clients, staff, and the public
- Strong interpersonal relationship building skills, with the ability to work collaboratively as a member of an interdisciplinary team
- Ability to problem-solve and adapt nursing interventions as needed
- Effective time management skills, with the ability to plan and organize daily work activities independently
Working Conditions and Physical Demands:
This role requires a detail-oriented approach in a dynamic environment, with physical activity including lifting, carrying, bending, reaching, kneeling, and other movements that emphasize good body mechanics. Individuals in this role are required to walk, sit, stand, and climb stairs throughout the day, with some tasks requiring fine hand movements.
