About the Role:
Closing the Gap Healthcare is seeking an Occupational Therapist to join our team of healthcare professionals. As an Occupational Therapist, you will be responsible for assessing and developing therapy care plans for clients in their homes. Your integrated approach to therapy will help clients improve their well-being and independence.
Key Responsibilities:
- Assess clients' needs and develop personalized therapy plans
- Provide therapy services in clients' homes, using a hybrid model of care that includes face-to-face and virtual sessions
- Collaborate with healthcare teams to build holistic treatment goals and service plans
- Use critical thinking to provide complex preventative or corrective actions for clients
Requirements:
- Graduate of a recognized Post-Secondary program in Occupational Therapy
- Current active registration and good standing as an Occupational Therapist with the College of Occupational Therapists of Ontario
- Valid Driver's License and reliable vehicle
