Job Summary:

Closing the Gap Healthcare Group is seeking an Occupational Therapist to join our team of healthcare professionals. As an Occupational Therapist, you will work with clients in their homes, providing therapy services to improve their well-being and independence.

Key Responsibilities:

  • Assess and develop therapy care plans for clients in their homes
  • Provide therapy services to clients, including developing health promotion programs, prescribing equipment and supplies, and promoting client safety
  • Collaborate with healthcare teams to build inter-discipline, holistic treatment goals and service plans
  • Work with clients and their families to identify and address barriers to independence

Requirements:

  • Graduate of a recognized Post Secondary program in Occupational Therapy
  • Current active registration and good standing as an Occupational Therapist with the College of Occupational Therapists of Ontario
  • Valid driver's license and reliable vehicle
