Job Title: Speech-Language Pathologist, Casual, Stroke Rehab, Labelle

About the Role:

We are seeking a highly skilled and experienced Speech-Language Pathologist to join our team at Home and Community Care Support Services. As a Speech-Language Pathologist, you will play a vital role in providing high-quality care to our patients, assessing and determining their care needs, and developing personalized treatment plans to help them achieve their goals.

Key Responsibilities:

  • Assess and determine patient care needs and eligibility
  • Provide access and referrals to community services
  • Engage with patients, caregivers, and other healthcare practitioners
  • Develop and implement treatment plans to help patients achieve their goals
  • Provide recommendations for follow-up and management of patient conditions
  • Collaborate with multidisciplinary teams to provide comprehensive care
  • Maintain accurate and up-to-date client records
  • Participate in professional development activities to stay current with best practices

Requirements:

  • Master's degree in Speech Therapy and current certificate of registration with the College of Audiologists and Speech-Language Pathologists of Ontario
  • Related professional experience in a SLP role
  • Knowledge of healthcare system and the role of Home and Community Care Support Services
  • Knowledge of relevant healthcare legislation and policies
