About the Role:

We are seeking a highly skilled and experienced Speech-Language Pathologist to join our team at Home and Community Care Support Services. As a Speech-Language Pathologist, you will play a vital role in providing high-quality patient care and support to individuals in need.

Key Responsibilities:

  • Assess and determine patient care needs and eligibility for speech therapy services.
  • Provide access and referrals to community services and resources.
  • Engage with patients, caregivers, and other healthcare practitioners to develop and implement effective treatment plans.
  • Conduct clinical assessments, develop treatment plans, and provide speech therapy services to patients in their homes, schools, and nursing homes.
  • Collaborate with multidisciplinary teams to ensure comprehensive patient care and support.
  • Maintain accurate and up-to-date client records and reports.
  • Participate in professional development activities and stay current with best practices in speech-language pathology.

Requirements:

  • Master's degree in Speech Therapy and current certificate of registration with the College of Audiologists and Speech-Language Pathologists of Ontario.
  • Related professional experience in a SLP role, preferably in a community-based setting.
  • Valid driver's license and access to a vehicle.
  • Fluency in English and French is an asset.
  • Vulnerable Sector Check completed within the last six months.
