Job Summary

We are seeking a Licensed Practical Nurse (LPN) to join our team as a Medical Care Provider in Home and Community settings. As an LPN with VON Canada, you will have the opportunity to work with diverse populations, providing medical care and support to individuals in their homes and communities.

Key Responsibilities

  • Provide medical care and support to individuals in their homes and communities, including diabetes management, dialysis, medication management, palliative care, phlebotomy, and wound care.
  • Work collaboratively with healthcare teams to develop and implement individualized care plans.
  • Conduct thorough assessments and evaluations to identify client needs and develop effective care strategies.
  • Provide education and support to clients and their families on health-related topics and disease management.
  • Collaborate with community resources and healthcare providers to ensure seamless care coordination.

Requirements

  • Current licensure as a Licensed Practical Nurse in the province of practice.
  • Experience in community health an asset.
  • A valid driver's license and access to a reliable vehicle required.
  • A current and original copy of a satisfactory Criminal Records Check required, and a Vulnerable Sector Search and/or a Child Abuse Registry Check may be required.
  • Ability to wear Personal Protective Equipment (PPE).
