Closing the Gap Healthcare Group is seeking a skilled Speech Language Pathologist to join our team as a Home Care Specialist. As a Speech Language Pathologist with our company, you will work independently with adult clients in their home settings, providing face-to-face therapy services.
Key Responsibilities:
- Conduct assessments in home settings to identify presenting impairments or diagnoses using standardized assessment tools.
- Develop and implement care plans using evidence-based information, relevant best practice guidelines, and clinical expertise.
- Collaborate with clients' family members, caregivers, and other healthcare professionals to ensure comprehensive care.

Requirements:
- Graduate of a recognized post-secondary program in Speech Language Pathology
