Job Summary:

Closing the Gap Healthcare Group is seeking a skilled Speech Language Pathologist to join our team in providing home-based services to adult clients. As a Speech Language Pathologist, you will work independently with clients in their home settings, conducting assessments and developing care plans to improve their communication and swallowing abilities.

Key Responsibilities:

  • Conduct assessments in home settings using standardized assessment tools to identify client needs and develop care plans.
  • Work with clients, family members, and caregivers to establish and implement care plans that promote effective communication and swallowing abilities.
  • Partner with healthcare professionals to ensure comprehensive care and support for clients.

Requirements:

  • Graduate of a recognized Post-Secondary program in Speech Language Pathology.
  • Current active registration in good standing with the College of Audiologists and Speech-Language Pathologists of Ontario.
  • Valid Driver's License and reliable vehicle.
  • Excellent communication and analytical skills, with strong organizational abilities.
