Job Title: Personal Support Worker - Nursing Care

We are seeking a compassionate and dedicated Personal Support Worker to join our team at Transitions Care, Inc. as a Nursing Care Specialist. In this role, you will provide high-quality care and support to our patients in their homes and communities.

Key Responsibilities:
  • Assist patients with daily living activities, such as bathing, dressing, and grooming.
  • Provide emotional support and companionship to patients.
  • Assist with medication management and administration.
  • Perform light housekeeping and laundry duties.
  • Assist with meal preparation and feeding patients.
  • Provide transportation to appointments and outings.
  • Assist with personal care, such as toileting and incontinence care.
  • Monitor and report any changes in patient condition to the healthcare team.
Requirements:
  • High school diploma or equivalent required.
  • First Aid and CPR certification required.
  • Experience working with seniors or individuals with disabilities an asset.
  • Ability to work independently and as part of a team.
  • Excellent communication and interpersonal skills.
