Job Summary

We are seeking a highly skilled and experienced Registered Nurse to join our team as a Patient Care Coordinator. This is a key leadership role that requires strong clinical expertise, excellent communication skills, and the ability to work collaboratively with a multidisciplinary team.

Key Responsibilities
  1. Provide clinical supervision and guidance to nursing staff, ensuring high-quality patient care and adherence to established standards and protocols.
  2. Lead and facilitate daily patient flow, discharge planning, and patient placement/referral processes, ensuring optimal patient outcomes and efficient use of resources.
  3. Supervise and coordinate the work of the clinical team, monitoring staff workload and patient factors to adjust patient assignments and ensure service levels are met.
  4. Support quality patient care and excellence in practice by promoting interprofessional collaboration, staff education, and best clinical practices.
  5. Initiate and lead plan of care conferences and rounds to optimize patient care management and ensure seamless communication among team members.
  6. Identify learning requirements of staff and provide input into the development of learning objectives, ensuring staff are equipped to meet changing practice needs.
  7. Act as a point person for patients and families, addressing concerns and questions regarding their care, and collaborating with Quality Improvement to implement corrective actions as needed.
  8. Participate in quality improvement and risk management activities, evaluating the effectiveness of changes in practice and generating recommendations for alternative approaches.
Requirements
  • Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
  • Three (3) years of recent related clinical experience in the designated clinical area, including one (1) year of supervisory experience.
  • Completion of post-graduate coursework and/or certification applicable to the designated clinical area.
  • Clinical proficiency/expertise in the designated clinical area.
  • Ability to supervise and provide leadership and work direction.
  • Ability to coordinate patient assignments, in collaboration with staff.
  • Ability to monitor staff workload and patient factors and adjust patient assignments accordingly.
