Job Summary

Fraser Health is seeking a highly skilled Patient Care Coordinator to join our team. As a key member of our mental health team, you will provide clinical supervision, guidance, and support to nursing staff and other clinical team members. Your expertise will be instrumental in ensuring the delivery of high-quality patient care and promoting a culture of excellence in practice.

Key Responsibilities
  • Provide clinical supervision and guidance to nursing staff and other clinical team members
  • Lead and facilitate daily patient flow, ensuring timely and effective patient care
  • Develop and implement individual care plans to optimize patient progression to discharge
  • Supervise and coordinate the work of the clinical team, ensuring quality patient care and safety
  • Monitor and address performance issues, providing coaching and mentoring to staff
  • Collaborate with internal and external stakeholders to promote interprofessional collaboration and team building
  • Participate in quality improvement and risk management activities, identifying opportunities for improvement and implementing changes
Requirements
  • Current practicing registration as a Registered Nurse and/or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives
  • Three years of recent related clinical experience in the designated clinical area
  • Ability to supervise and provide leadership and work direction
  • Ability to coordinate patient assignments and monitor staff workload and patient factors
  • Ability to establish priorities for patient care and daily operations
  • Ability to communicate effectively verbally and in writing
