Overview of the Grande Prairie Primary Care Network
">The Grande Prairie Primary Care Network (GP PCN) was formed to enhance the delivery of primary healthcare services. Our network provides innovative and collaborative initiatives that improve communication, increase coordination of services, and enhance medical clinic practices.
Position Summary:
">This part-time Family Practice Nurse role will work collaboratively within a family practice environment to deliver high-quality primary care services. The successful candidate will work in alignment with the Patient's Medical Home Model of Care, supporting comprehensive team-based care under the supervision and leadership of primary care physicians.
Responsibilities:

- Facilitate continuity of care and support patients through their healthcare journey.">
- Coordinate and manage patient care within the scope of practice.">
- Collaborate effectively with primary care physicians, PCN team members, and other healthcare professionals to provide seamless care.">
- Develop and maintain professional knowledge and skills, identifying areas for improvement and implementing changes as needed.">
- Contribute to quality improvement activities and participate in reviewing and providing feedback on clinical policies and protocols.">
Required Skills and Qualifications:
">To succeed in this role, you must possess strong communication and interpersonal skills, with the ability to build relationships with patients, families, and healthcare professionals. You should have fluency in English, both written and verbal, and be proficient in computer skills, including email, MS Word, and EMR software.
">You must also hold a valid CLPNA license and current CPR certification, as well as a DCS or DEC education. A minimum of 5 years' experience in primary care, acute care, or community development is preferred.
">Benefits and Compensation:
">This part-time position offers a competitive hourly rate of $32.83 per hour, with a guaranteed 30 hours per week. Benefits include dental care, disability insurance, employee assistance program, extended health care, RRSP match, and vision care.
