Job Summary
Vancouver Coastal Health is seeking a skilled Intake Nurse Specialist to join our team at the Squanish Community Health Centre. As an Intake Nurse Specialist, you will play a critical role in conducting centralized intake functions, ensuring seamless linkages between clients and appropriate resources.
Key Responsibilities
- Conduct initial screenings, triage, and referrals based on client needs and best practices.
- Provide consultation and education to clients and families, ensuring they receive the necessary support and resources.
- Establish and maintain relationships with healthcare providers, community resources, and other stakeholders to ensure effective communication and collaboration.
- Prepare and maintain accurate client records, intake/referral data, and statistics.
- Participate in team meetings, continuing education, and quality improvement initiatives to enhance patient care and outcomes.
Qualifications
To be successful in this role, you will require:
- A current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- Three (3) years of recent related experience in the designated clinical program area or an equivalent combination of education, training, and experience.
- Broad knowledge of nursing theory and practice within a client and family-centered care model.
- Demonstrated ability to provide client-focused, trauma-informed, harm reduction, and culturally safe care.
- Knowledge of provincial acts, regulations, and program policies and guidelines related to home and community care.
- Ability to operate related equipment and computerized client care information systems.
