Job Summary
Vancouver Coastal Health is seeking a Registered Psychiatric Nurse or Registered Nurse to join the Home Care Nursing team at Squanish Community Health Centre in Vancouver.
Key Responsibilities
- Conduct centralized intake functions to ensure linkages between clients and resources.
- Perform initial screening, triage, and referrals based on client needs and best practices.
- Provide information to health providers and the public, establish relationships, and maintain accurate records.
Qualifications
- Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives.
- Three years of recent experience in the designated clinical program area or an equivalent combination of education and experience.
Knowledge and Skills
- Broad knowledge of nursing theory and practice within a client-centered care model.
- Knowledge of provincial acts, regulations, and program policies related to home and community care.
- Ability to communicate effectively and work collaboratively as a member of a multidisciplinary team.
