Estimated Salary: $60,000 - $80,000 per year
Job Description:
We are seeking a Registered Nurse to join our team as a Nursing Care Coordinator. As a key member of our care team, you will be responsible for ensuring that our patients receive the highest level of care possible.
Responsibilities:
- Develop and implement individualized care plans for patients based on their specific needs and goals;
- Provide direct patient care, including medication administration, wound care, and other treatments as needed;
- Assess and monitor patient progress, making adjustments to care plans as necessary;
- Collaborate with other healthcare professionals to ensure seamless care coordination;
- Document patient information and maintain accurate records.
Requirements:
- Degree in Nursing (BScN or ADN);
- Current registration with the relevant provincial nursing association;
- At least 2 years of experience in a related field;
- Excellent communication and interpersonal skills;
- Ability to work independently and as part of a team.
