About the Role

We are seeking a skilled Social Worker to join our Hospice Palliative Community Services team in Tri Cities. As a key member of our interdisciplinary team, you will provide comprehensive therapeutic and clinical counseling services to clients and families experiencing end-of-life issues.

Key Responsibilities
  • Conduct social and psychosocial assessments of clients and families
  • Provide grief and bereavement services to clients registered with the Hospice Palliative Care Program
  • Collaborate with colleagues, clients, and caregivers to develop a comprehensive plan of care
  • Act as a resource for clients and their families by providing information regarding community resources and access to these resources
  • Participate in quality improvement activities by providing input into the development, implementation, and evaluation of program goals, objectives, policies, and procedures
Requirements
  • Master's degree in Social Work from an approved School of Social Work
  • Two (2) years' recent, related clinical experience working with palliative clients
  • Current full registration with the BC College of Social Workers
  • Valid BC Driver's license and access to personal vehicle for business-related purposes
