Job Title: Social Worker, Renal

About the Role:

We are seeking a highly skilled and experienced Social Worker to join our Renal team at Fraser Health. As a key member of our interdisciplinary team, you will play a vital role in providing comprehensive social work services to patients and families affected by renal disease.

Key Responsibilities:

  • Conduct comprehensive social work assessments of patients and families to identify their needs and develop personalized care plans.
  • Provide counseling and therapy services to patients and families to address emotional, psychological, and social challenges related to renal disease.
  • Collaborate with healthcare professionals and community agencies to ensure seamless care coordination and access to resources.
  • Develop and deliver education programs and workshops to patients, families, and healthcare professionals on renal disease management and self-care.
  • Participate in research activities and program planning to improve care and services for renal patients and families.

Qualifications:

  • Master's degree in Social Work from an approved school of Social Work.
  • Two years' recent related clinical social work experience working with renal patients and families in an acute care and/or community environment.
  • Current full registration with the BC College of Social Workers.
