Treatment Coordinator

Job Summary:
The Recovery Navigator at Phoenix Society plays a pivotal role in empowering clients to reach their full potential and overcome addiction challenges.
This position requires an individual who can work effectively as part of a multi-disciplinary team, with a focus on supporting the Youth and Young Adult Population within our treatment programs.
Main Responsibilities:
- Contribute to maintaining and continuously improving the intake, admissions, and orientation process through collaborative team efforts.
- Maintain client confidentiality while responding to their rights to healthcare information.
- Evaluate clients' suitability for the program and refer them to other agencies when necessary.
- Work with the integrated addiction services team to develop case plans and address client concerns, seeking assistance when required and communicating with relevant professionals.
- Ensure accurate record-keeping of client data, including assessment results, care plans, goal setting, case notes, and discharge summaries.
- Demonstrate leadership skills by serving as a positive role model, coach, advisor, and rational authority for clients.
- Assist new clients in identifying acute and chronic mental health issues and collaborate with staff members to address these concerns.
- Develop treatment plans and strategies tailored to meet individual client needs in consultation with primary treatment providers.
- Support clients in completing daily activities, such as household upkeep and community participation.
- Monitor client behavior, document any changes, and report physical or mental health updates to the clinical team.
Required Qualifications:
- A minimum of Grade 12 and graduation from a recognized Mental Health Care Worker Program or equivalent combination of education, training, and experience.
- At least one year of experience working with vulnerable populations.
- A valid Class 5 driver's license with a clean driving record.
- CPR certification and First Aid training.
- Foodsafe certification and completion of the Employee Immunization Record form.
Compensation and Benefits:
The estimated annual salary for this role is between $57,984 and $61,440, based on an hourly rate of $27.92-$29.60 per hour.
