Job Summary

The Access Discharge Coordinator plays a critical role in ensuring seamless transitions for patients with complex mental health and substance use challenges. This position is responsible for coordinating referrals, developing and implementing access utilization systems, and working collaboratively with various stakeholders to promote efficient bed utilization.

Key Responsibilities

  • Coordinate and maintain a centralized referral registry and access utilization system for designated mental health and/or addiction services.
  • Develop and determine the effectiveness of registry and access system policies, procedures, and standards in collaboration with acute psychiatric mental health care services, community mental health and addiction teams, and other related programs.
  • Receive and review referrals for access and discharge to mental health and/or addiction services, identifying suitable services in accordance with established protocols.
  • Monitor referrals from acute mental health and medical sites, working collaboratively with various sites and/or health authorities, mental health and/or addiction teams, and other related programs to ensure efficient flow of referrals and promote maximum bed utilization.
  • Provide consultation and guidance to mental health and addiction teams, acute sites, other referral sources, families, and service users on the interpretation and application of policies relevant to eligibility and access to bed resources.

Requirements

  • Graduation from an approved School of Nursing with current practicing registration as an RN or RPN with the British Columbia College of Nurses and Midwives (BCCNM).
  • Two years' recent, related nursing experience relevant to the designated client population or an equivalent combination of education, training, and experience.
  • Valid BC Driver's License. Local area travel may require the use of a personal vehicle.

Preferred Qualifications

  • Nursing science theory and practice within a client and family-centered model of care.
  • British Columbia College of Nurses and Midwives (BCCNM) standards for Nursing Practice.
  • Adult and older adult residential care, tertiary, acute, and community mental health and/or addiction services and related issues.
  • Relevant policies, procedures, and standards of care.
  • Other health care disciplines and their role in patient care.
  • Awareness of and commitment to learning and understanding the Truth & Reconciliation Commission's 94 Calls to Action (2015), In Plain Sight Report (2020), BC's Declaration on the Rights of Indigenous Peoples Act (2019), and other related reports.
  • Commitment to upholding the shared responsibility of creating lasting and meaningful reconciliation in Canada as per TRC (2015) and BC's Declaration on the Rights of Indigenous Peoples Act (2019).
