Job Summary

We are seeking a highly skilled Access Discharge Coordinator to join our team at BC Mental Health and Substance Use Services. As a key member of our team, you will play a critical role in coordinating access to mental health and addiction services, ensuring that clients receive the care they need in a timely and efficient manner.

Key Responsibilities

  • Coordinate and develop a centralized referral registry and access utilization system for mental health and addiction services.
  • Work closely with referring agencies to ensure referrals are prioritized in accordance with established policies and procedures.
  • Receive and review referrals for access and discharge to mental health and addiction services, identifying suitable services for referral in accordance with established protocols.
  • Monitor referrals from acute mental health and medical sites, working collaboratively with various sites and health authorities to ensure efficient flow of referrals and promote maximum bed utilization.
  • Provide consultation and guidance to mental health and addiction teams, acute sites, and other referral sources on the interpretation and application of policies relevant to eligibility and access to bed resources.

Requirements

  • Graduation from an approved School of Nursing with current practicing registration as an RN or RPN with the British Columbia College of Nurses and Midwives (BCCNM).
  • Two years' recent, related nursing experience relevant to the designated client population or an equivalent combination of education, training, and experience.
  • Valid BC Driver's License. Local area travel may require the use of a personal vehicle.
