Job Summary

The Licensed Practical Nurse (LPN) at Crosstown Clinic is responsible for providing high-quality nursing care to patients/residents in accordance with established policies and procedures. The LPN works collaboratively with the interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient/resident.

Key Responsibilities

  • Conduct thorough assessments of patients/residents to identify their physical, social, spiritual, and psychological needs.
  • Develop and implement care plans that address the identified needs and promote optimal patient/resident outcomes.
  • Provide safe and respectful nursing care to patients/residents, including administering medications, performing procedures, and responding to emergencies.
  • Communicate effectively with patients/residents, families, and healthcare team members to ensure seamless care coordination.
  • Participate in care conferences and contribute to the development of individualized care plans.

Requirements

  • Graduation from a recognized program for Practical Nurses or an equivalent combination of education, training, and experience.
  • Registration with the British Columbia College of Nurses and Midwives as a practicing LPN registrant.
  • Current basic cardiac life support certification.
