Job Summary

CareRx is seeking a highly skilled Clinical Pharmacist to join our team in a remote opportunity. As a Clinical Pharmacist, you will play a vital role in ensuring continuity of care for clients in long-term care, supportive living, and other congregate living communities.

Key Responsibilities

  • Conduct medication reconciliation in accordance with CareRx policies and procedures to improve patient outcomes.
  • Provide comprehensive pharmaceutical care in collaboration with the interdisciplinary team and offer recommendations to clinicians.
  • Work with the healthcare team to develop strategies to optimize resident care in transition and improve outcomes.
  • Produce quality metrics related to clients and medications in transitions and support departmental initiatives to deliver quality patient care.
  • Liaise between the dispensing pharmacy, manager, and staff to ensure optimal pharmacy services are provided to each facility.

Requirements

  • Registration in good standing with the Ontario College of Pharmacists (RPh).
  • Prior experience in long-term care pharmacy or hospital pharmacy.
  • Ability to effectively use technology to support medication management, monitoring, and documentation.
  • Excellent written and verbal communication, interpersonal, and presentation skills.
  • Proven analytical and problem-solving experience.
  • Ability to organize and prioritize work effectively.
  • Self-motivation and results-oriented.
  • Comfortable working in a 100% paperless environment.
