Job Title: Pharmacist

Baycrest is seeking a highly skilled and experienced Pharmacist to join our team. As a key member of our interdisciplinary team, you will play a vital role in ensuring the safe and effective use of medications across our hospital programs.

Key Responsibilities:
  • Participate in rounds and care conferences to ensure safe prescribing decisions and monitor patient outcomes.
  • Identify, prevent, and resolve drug-related problems through medication reviews and patient care planning.
  • Ensure seamless medication reconciliation during patient transfers and discharges.
  • Prepare and dispense medications accurately and efficiently.
  • Participate in departmental medication management projects and Pharmacy clinical programs.
  • Provide education to Pharmacy students and oversee Pharmacy Technicians.
  • Implement and maintain best practices for medication safety and patient safety.
Qualifications:
  • Current and valid license to practice as a Pharmacist from the Ontario College of Pharmacists (Part A).
  • Minimum of 2 years of hospital pharmacy experience.
  • Hospital Pharmacy Residency or Doctor of Pharmacy Degree (completed or in progress).
  • Demonstrated expertise in pharmaceutical care.
  • Excellent communication, organization, and critical thinking skills.
  • Computer skills, including Microsoft Office and Meditech experience.
