Job Summary

The University Health Network is seeking a skilled Pharmacist to join our team. As a Pharmacist, you will play a critical role in ensuring the safe and effective use of medications in our healthcare setting.

Key Responsibilities

  • Review and dispense medications to ensure appropriateness and accuracy
  • Identify and resolve medication-related problems
  • Monitor medication therapy for effectiveness and toxicity
  • Conduct medication reconciliation to ensure accurate medication lists
  • Provide education and guidance to patients and healthcare staff on medication use
  • Collaborate with healthcare teams to optimize medication use and patient outcomes

Requirements

  • Completion of a university degree in pharmacy or equivalent
  • Active registration with the Ontario College of Pharmacists
  • Experience working in a healthcare setting, preferably in a pharmacy
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
