Job Title: Personal Support Worker - Nursing Care

We are seeking a compassionate and dedicated Personal Support Worker to join our team at Transitions Care, Inc. as a Nursing Care Specialist. In this role, you will provide high-quality care and support to our patients in their homes, ensuring their comfort, safety, and well-being.

Key Responsibilities:
  • Accompany patients on outdoor recreational activities to promote physical and mental well-being.
  • Assist with cleaning, sterilizing, setting up, and assembling medical equipment to maintain a safe and hygienic environment.
  • Perform other duties related to patient care and comfort, including bathing, dressing, and grooming.
  • Supervise patients' exercise routines to promote physical activity and mobility.
  • Maintain inventory of supplies to ensure a smooth operation of patient care services.
  • Deliver messages, reports, requisitions, and specimens between departments to facilitate effective communication.
  • Make beds and maintain patients' rooms to ensure a clean and comfortable environment.
  • Transport patients between care facilities and in wheelchairs or stretchers to ensure safe and efficient transportation.
  • Answer call signals to determine patients' needs and provide timely assistance.
  • Collect specimens and administer first aid in emergency situations to ensure patient safety.
Requirements:
  • Secondary (high) school graduation certificate.
  • Will train for the role.
Work Environment:
  • Home care and visiting care agency.
  • Various locations.
