Job Summary

The Community Health Facilitator plays a vital role in coordinating and facilitating client service delivery, ensuring seamless access to team resources. This position requires excellent organizational and facilitation skills, with a strong focus on results and alignment with organizational values.

Key Responsibilities

  • Facilitate the intake of new clients, including program eligibility and initial assessment
  • Identify and access appropriate resources for clients outside the team
  • Coordinate program activities with the team
  • Provide competent services to clients in accordance with required standards and policies
  • Provide community education and foster understanding in the community

Requirements

  • Post-secondary degree or diploma in Health, Social Services, Community Development, or a related field
  • Minimum of five years of client service experience in community development, health, or social services
  • Excellent organizational and facilitation skills

Preferred Qualifications

  • Minimum of three years working in/with an interdisciplinary team
  • Valid class 5 driver's license and access to a reliable vehicle
