Job Summary

We are seeking a skilled Community Health Facilitator to join our team at Wiisocotatiwin. The successful candidate will be responsible for facilitating the intake of new clients, identifying and accessing appropriate resources, and providing competent services in accordance with required standards and policies.

Key Responsibilities

  • Facilitate the intake of new clients, including program eligibility and initial assessment
  • Identify and access appropriate resources for clients outside the team
  • Coordinate program activities with the team as directed
  • Provide competent services to clients in accordance with required standards and policies

Requirements

  • Post-Secondary degree or diploma in Health, Social Services, Community Development, or related field
  • Minimum of five years client service experience in community development, health, or social services
  • Excellent organizational and facilitation skills
