Job Title: Social Worker

We are seeking a highly skilled and compassionate Social Worker to join our team at William Osler Health System.

Job Summary:

The successful candidate will be part of a multi-disciplinary team responsible for the care of patients and families within the Medicine program. As a Social Worker, you will work collaboratively with internal medicine specialists, primary care physicians, nursing, allied health team, and the patient/family to manage complex medical patients through psychosocial assessment, implementing a plan of care, monitoring and evaluating patient outcomes on an ongoing basis.

Key Responsibilities:
  • Provide psychosocial assessment and develop individualized plans of care for patients and families
  • Collaborate with healthcare teams to ensure comprehensive care and support
  • Provide education and support to patients and families on chronic disease management and health promotion
  • Participate in biweekly case management meetings to discuss patient progress and develop strategies for improvement
  • Develop and maintain relationships with community resources and services to support patient needs
  • Stay up-to-date with best practices and research in social work and healthcare
Qualifications:
  • Masters degree in social work required
  • Three to five years experience working within a medical unit providing counselling for patients and families
  • Knowledge of community resources and legislation related to healthcare
  • Excellent communication and interpersonal skills
  • Ability to work as part of a multi-disciplinary team
  • Computer proficiency preferred
