Job Title: Social Worker

We are seeking a highly skilled and compassionate Social Worker to join our team at William Osler Health System. As a Social Worker, you will play a vital role in providing psychosocial support to patients and families within our Medicine program.

Key Responsibilities:
  • Collaborate with internal medicine specialists, primary care physicians, nursing, and allied health teams to manage complex medical patients through psychosocial assessment, implementing a plan of care, monitoring, and evaluating patient outcomes.
  • Provide care in a compassionate and culturally sensitive manner, ensuring a positive patient experience.
  • Participate in biweekly case management meetings to discuss clinic patients, including plan of care, follow-up, and evaluation.
  • Provide health teaching to patients and families, including thorough discharge instructions to ensure continuity of care in the community.
  • Maintain best practice through clinical research and literature review.
  • Participate in program development and quality improvement initiatives.
Qualifications:
  • Masters degree in social work required.
  • Three to five years of experience working within a medical unit providing counseling for patients and families.
  • Knowledge of community resources and relevant legislation.
  • Excellent communication and interpersonal skills.
  • Ability to work as part of a multi-disciplinary team.
  • Computer proficiency preferred.
