We are seeking a highly skilled and compassionate Reminiscence/Memory Care Coordinator to join our team at Sunrise Senior Living. As a key member of our care team, you will be responsible for providing exceptional care and support to our residents with dementia and their families.
Key Responsibilities- Develop and implement individualized care plans that promote the physical, emotional, and social well-being of our residents.
- Lead and coordinate reminiscence programs and activities that stimulate the minds and engage the hearts of our residents.
- Collaborate with our care team to ensure seamless delivery of care services and support to our residents and their families.
- Conduct regular assessments and evaluations to identify the changing needs of our residents and make recommendations for adjustments to their care plans.
- Provide education and support to our residents and their families on dementia care and management.
- Participate in quality improvement initiatives to enhance the overall care experience for our residents.
- College degree in a related field (e.g., gerontology, social work, nursing).
- Minimum one year of experience in dementia care or a related field.
- Strong knowledge of dementia care principles and practices.
- Excellent communication and interpersonal skills.
- Ability to work collaboratively as part of a team.
- Flexibility to work varied shifts, including weekends and holidays.
