Clinical Social Worker

We are seeking a highly skilled Clinical Social Worker to join our team at Fraser Health Authority. As a Clinical Social Worker, you will play a vital role in providing social work services to improve the emotional and social consequences of a health condition for our clients and their families.
Job Summary:The successful candidate will be responsible for promoting client/family autonomy, identifying and engaging with eligible clients, providing therapeutic counseling services, and facilitating referrals to community resources. You will work collaboratively with our interdisciplinary team to develop and implement comprehensive care plans that address the unique needs of our clients.
Responsibilities:- Conduct social and psychosocial assessments of patients and families to identify their needs and develop effective care plans.
- Provide therapeutic counseling services to clients and families experiencing a variety of circumstances related to their illness.
- Facilitate a process whereby patients and families identify their needs for recovery, adaptation, and mastery of their situation.
- Follow up and provide ongoing monitoring of psychosocial situations for clients and evaluate their response to interventions.
- Participate in quality improvement activities by providing input into the development, implementation, and evaluation of program goals, objectives, policies, and procedures.
- Bachelor's degree in Social Work from an approved school of Social Work.
- Two (2) years' recent related clinical social work experience working with individuals, groups, and families.
- Current full registration with the BC College of Social Workers.
