We are seeking a highly skilled and compassionate Access Navigator to join our Provincial Preschool Autism Service team. As a key member of our clinical intake and care coordination stream, you will be the first point of contact for families contacting our service. Your primary responsibility will be to explore child and family needs, match them with available services, and provide navigation and coordination support throughout their preschool years.
Key Responsibilities- Conduct virtual and telephone-facilitated intake to gather and provide information and support to patients and families
- Match child and family needs with available services, including clinical streams and community-based organizations
- Develop and maintain professional relationships with families throughout their preschool years
- Support access to school-age diagnostic services as needed
- Participate in planning, development, and ongoing evaluation/quality improvement
- Bachelor of Social Work or equivalent health care professional degree
- Registration with the Nova Scotia College of Social Workers or applicable College
- Minimum of two years related clinical experience
- Three to five years' experience in intake and/or client navigation preferred
- Experience in pediatric settings and specifically autism preferred
