We are seeking a skilled and compassionate Registered Practical Nurse to join our team in the community nursing role.

Your Responsibilities:
- Participate in scheduled visits to assess and address patient needs within your scope of practice.
- Provide ethical and safe nursing care, utilizing appropriate assessment tools and techniques.
- Develop and continuously monitor care plans, making adjustments as required.
- Engage patients using a collaborative approach, building rapport and trust.
- Maintain and update professional patient records related to encounters and plans.
Requirements:
- Registration with the CNO as a Registered Practical Nurse in good standing.
- A valid driver's license and access to a vehicle, with reliable transportation.
- Experience in delivering community care is an asset.
- Reliability, flexibility, and a willingness to travel to and from patient residences within a designated area.
- Experience working with vulnerable populations is beneficial.
- A passion for making a difference in your community.
- Required on-call coverage, including after-hours, to ensure patient safety and meet care needs.
Estimated Salary: $65,000 - $80,000 per annum, based on location and experience.
