About the Role:

We are seeking a dedicated Home Health Nurse to join our team at ParaMed. In this role, you will provide high-quality care to patients in their homes, working with patients of all ages and complexity of care needs.

Key Responsibilities:

  • Assess patients' individualized needs using appropriate tools and techniques.
  • Execute and continuously monitor care plans to ensure optimal patient outcomes.
  • Engage patients in a collaborative approach, building rapport and trust to deliver exceptional care.
  • Maintain and update professional patient records related to patient encounters and plans.

Requirements:

  • Registration with the CNO as a Registered Nurse in good standing.
  • Valid driver's license and access to a vehicle.
  • Experience in delivering community care is an asset.
  • Reliability and flexibility to meet the needs of patients.
  • Willingness to travel to and from patient residences within a designated area.
  • Passion for making a difference in your community.
