Job Summary

The Home Support Worker provides personalized care and support services to clients and families within the framework of VON Canada's policies, procedures, and standards. This role involves collaborating with healthcare professionals to meet the needs of clients and promote their independence and well-being.

Key Responsibilities
  • Conduct thorough assessments of clients' homes, personal situations, and care plans to identify areas for improvement and ensure their needs are met.
  • Develop and implement personalized care plans under the guidance of the Personal Support Program Manager, ensuring safety, liability, and financial considerations.
  • Provide personal care services, including light cleaning, meal preparation, and household management, as directed by the Personal Support Program Manager or immediate supervisor.
  • Communicate effectively with clients, families, and healthcare professionals to ensure seamless care coordination and address any concerns or changes in client needs.
  • Perform daily call checks and complete required forms to maintain efficient service delivery.
  • Assess clients' communication patterns and abilities to provide tailored support.
  • Function in accordance with VON Canada's risk management policies, procedures, and guidelines to promote client safety.
Requirements
  • High school diploma or equivalent.
  • CCA Certification (Nova Scotia) or equivalent.
  • Must have a certificate in Home Support Worker (Level I, II, III), Personal Support Worker program, or Health Care Aide.
  • Current Standard First Aid Certificate or CPR level C Certificate, if applicable.
  • Some household management experience and experience working with elderly or ill individuals or children.
Skills and Abilities
  • Strong communication and interpersonal skills.
  • Ability to work effectively in a multidisciplinary team or independently.
  • Good assessment skills and awareness of community resources.
  • Ability to take direction well and adjust to different social and economic environments.
  • Capacity to maintain confidentiality and adhere to strict confidentiality standards.
Work Conditions and Physical Capabilities

This role requires working in a fast-paced environment, with attention to detail, and the ability to lift and carry using appropriate lifting techniques. The Home Support Worker must be able to walk, sit, stand, climb stairs, and perform fine hand movements.
