Mental Health Outreach Coordinator

Vancouver Coastal Health is seeking a skilled Substance Use Outreach Clinician to join our team at Powell River Mental Health. As a key member of our Aftercare Clinicians team, you will play a vital role in supporting individuals with substance use and mental health presentations.
Key Responsibilities- Develop and implement a strengths-based outreach model that incorporates harm reduction approaches to support client goals and wellness initiatives.
- Provide education, group facilitation, and case management services to clients across the transtheoretical model of change.
- Establish and maintain relationships with clients, collaborating with multidisciplinary teams and community resources to facilitate clients' wellness journeys.
- Participate in committees and meetings, engage in ongoing team planning, development, and quality improvement activities, and maintain and update own knowledge.
- Provide direct clinical services, including immediate assessment, intervention, referral, and support to clients in urgent situations.
- Collaborate with colleagues and other caregivers to identify and resolve client care issues, ethical dilemmas, and coordinate and integrate care.
Powell River is a scenic coastal town on the Sunshine Coast of British Columbia, offering a unique blend of outdoor adventure and cultural amenities. The area provides endless opportunities for kayaking, canoeing, rock climbing, golfing, boating, hiking, biking, fishing, and scuba diving.
Qualifications- Master's Degree in a health science discipline from a recognized program.
- Two (2) years' recent experience practicing in community-based or Indigenous-led or First Nations organizations working with individuals with substance use and mental health presentations.
- Valid driver's license. Local area travel requires the use of a personal vehicle for which mileage reimbursement will be paid.
- Pursuant to Section 41 of the British Columbia Human Rights Code, preference will be given to candidates who identify as Indigenous, including First Nations, Inuit, and/or Metis.
- Demonstrate knowledge of substance-use recovery services in the community and across British Columbia.
- Demonstrate knowledge of health inequities and discrimination faced by designated client population.
- Demonstrate knowledge of the principles of harm reduction in a healthcare setting.
- Demonstrate knowledge of strengths-based, client-centered approaches and ability to incorporate into practice.
- Demonstrate knowledge of Indigenous Cultural Safety.
- Demonstrate knowledge of traditional healing practices.
- Demonstrate knowledge of trauma-informed practice.
- Demonstrate knowledge of substance-use recovery model.
- Knowledge of the principles of recovery in mental illness.
- Knowledge of Psychosocial Rehabilitation methodology.
- Broad knowledge of clinical theory and counseling skills (individual and group-based).
- Broad knowledge of mental illness and treatment.
- Broad knowledge of substance use and addictions treatment.
- Broad knowledge of psychopharmacology (indications and side-effects).
- Broad knowledge of psychoeducation principles and methods.
- Broad knowledge of other facilities and community resources.
- Broad knowledge of legislation and government policies and procedures that relate to client care, including the BC Mental Health Act.
- Knowledge of DSM IV Diagnostic Classification System, mental status examination, and case formulation.
- Demonstrated ability to conduct mental health and addictions assessments and provide treatment planning and crisis intervention.
- Demonstrated ability to communicate effectively, both verbally and in writing, with colleagues, clients, and their families, students, and service providers in health and social services.
- Demonstrated de-escalation and conflict resolution skills.
- Ability to manage aggressive and suicidal behavior.
- Ability to work with a culturally diverse population.
- Ability to conduct a risk assessment and respond appropriately.
- Demonstrated ability to establish workload priorities.
- Demonstrated ability to work both independently and collaboratively as a member of an interdisciplinary team.
