About the Role: In this role, you will work collaboratively with fellow professionals to develop and implement individualized program plans, provide nursing care and support, and ensure the health and safety of supported persons. You will also be responsible for communicating with staff, professionals, and families, and making clinical judgments based on current nursing standards.

Key Responsibilities:
  • Develop and implement individualized program plans that address the total needs of each supported person.
  • Provide nursing care and support to individuals with diverse abilities, ensuring their dignity, spirit, and individuality are respected and maintained.
  • Communicate with fellow professionals, staff, and families regarding assessments, required monitoring, necessary nursing care, and other relevant information.
  • Maintain a clean and tidy living environment at all times, and ensure the health and safety of supported persons.
