We are seeking a skilled and compassionate Social Worker to join our Renal team at Fraser Health Authority. As a key member of our interdisciplinary team, you will provide psycho-social assessments and treatment to clients and families in the Renal Program, promoting their quality of life and well-being.
Key Responsibilities- Conduct thorough psycho-social assessments to identify client needs and develop effective treatment plans.
- Provide clinical and supportive counseling services to clients and their families, addressing their concerns and promoting coping strategies.
- Offer education and guidance to individuals, couples, families, or groups on managing the psycho-social implications of treatment modalities and lifestyle changes.
- Collaborate with clients, families, and staff to access community and rehabilitation services, addressing financial, vocational, transportation, childcare, and accommodation concerns.
- Participate in quality improvement activities, contributing to the development, implementation, and evaluation of departmental and program goals, objectives, policies, and procedures.
- Prepare statistics and reports as required, in accordance with the workload measurement system.
- Engage in department or program-sponsored research, conducting literature searches, administering surveys, and analyzing data as needed.
- Bachelor's degree in Social Work from an approved school of Social Work.
- Two (2) years' recent related clinical experience working with clients in an acute care and/or community health care environment, or an equivalent combination of education, training, and experience.
- Current full registration with the BC College of Social Workers.
- Valid BC Driver's License and access to a personal vehicle for business-related purposes.
