Your primary responsibility will be to provide comprehensive case management services, including assessment, goal setting, and service coordination. You will also work closely with staff and clients to develop strategies that support clients in maintaining their housing and advocating on their behalf when needed.

Key Responsibilities
  • Provide comprehensive case management services, including assessment, personal goal setting, and service coordination.
  • Work closely with staff and clients to develop strategies that support clients in maintaining their housing and advocating on their behalf when needed.
  • Link clients to various community resources and facilitate referrals to other community support services as required.
  • Ensure the coordination of services and a continuum of care.

Requirements
  • Bachelor's degree in Social Work or a related discipline.
  • 3+ years of experience in mental health and harm reduction, housing support, and working with individuals who have experienced homelessness.
  • Effective communication and interpersonal skills.
  • Ability to work independently and as part of a team.
