As a Community Support Specialist, you will play a vital role in supporting individuals with disabilities and complex needs to achieve their goals and live independently.

Key Responsibilities

  • Provide support services to individuals with disabilities and complex needs to achieve their goals and live independently.
  • Develop and implement individualized care plans to meet the unique needs of each individual.
  • Support individuals in daily living skills, such as meal planning, preparation, and household cleaning.
  • Encourage and facilitate individuals to participate in community activities and events.
  • Provide transportation for individuals using IHACSS Fleet Vehicles.
  • Administer prescribed medications as directed.
  • Facilitate daily individual in care schedules as required.
  • Ensure all medical appointments and health concerns are followed through and documented accordingly.
  • Ensure Support Homes are kept clean, orderly, and all hazards are eliminated or controlled.
  • Ensure regular fire drill, smoke detector, and inspections of premises/equipment are completed.

Requirements

  • Degree or diploma in Health Services, Human Services, Disability Services, Correctional Services, or a relevant field.
  • 2 years of related experience working with the population served.
  • Possession of a valid driver's license and a reliable vehicle.
  • Clean driver abstracts and document proof of insurance.
  • Vulnerable sector check and child intervention record check.
  • Proficiency in English reading and writing skills.
  • Demonstrated aptitude in working with diverse populations.
  • Proficiency in collaborative teamwork.
  • Working knowledge and implementation of regulatory traffic laws.
  • Code of conduct of caregiver ethics and morals.
  • Flexibility to accommodate various shifts, including weekends and overnights.
