Job Title: Personal Support Worker - Reactivation Care Centre

Job Summary:

We are seeking a highly motivated and compassionate Personal Support Worker to join our team at North York General Hospital's Reactivation Care Centre. As a Personal Support Worker, you will play a vital role in providing exceptional care to our patients, promoting their independence, dignity, and well-being.

Key Responsibilities:

* Assist nursing personnel in providing personal care to residents, including hygiene, grooming, and dressing
* Perform direct and indirect patient care activities in accordance with the Interprofessional Care Plan
* Support patients' independence, dignity, social, emotional, and cultural well-being
* Communicate effectively with elderly people and colleagues

Requirements:

* Completed Personal Support Worker (PSW) certificate from a College of Applied Arts and Technology
* 1-2 years experience in inpatient Geriatric/Rehab or Long-Term Care experience
* Demonstrated Knowledge of Infection Control Practices
* Demonstrated aptitude to foster caring and trusting relationships with elderly patients and families
