Job Summary

We are seeking a skilled and compassionate Registered Nurse (RN) or Registered Practical Nurse (RPN) to join our team as a Community Health Nurse. As a Community Health Nurse, you will work with patients in their homes, clinics, and other community settings, providing exceptional nursing services and developing meaningful relationships with patients and their families.

Key Responsibilities
  • Develop and maintain effective care plans for patients in community health settings
  • Provide nursing services to patients in their homes, clinics, and other community settings
  • Communicate and liaise with patients, families, and other healthcare professionals to ensure seamless care coordination
  • Perform other clinical duties as required
Requirements
  • Current College of Nurses of Ontario (CNO) registration in good standing
  • Strong desire and commitment to making a difference in the lives of patients
  • Exceptional interpersonal skills
  • Valid driver's license and vehicle
  • Excellent English verbal and written communication skills
  • CPR Certification
  • Current immunization record, including 2-step TB test

About the Role
