Job SummaryWe are seeking a skilled and compassionate Registered Nurse (RN) or Registered Practical Nurse (RPN) to join our team as a Community Health Nurse. This is a full-time position based in Clinton, ON, with a focus on providing exceptional nursing services to patients in their homes, clinics, and other community settings.
Key Responsibilities- Develop and maintain effective care plans for patients receiving community health services
- Provide nursing services in a variety of settings, including in-home, clinic, and retirement home settings
- Communicate and liaise with patients, family members, and other healthcare professionals to ensure seamless care coordination
- Perform other clinical duties as required
- Current College of Nurses of Ontario (CNO) registration in good standing
- Strong desire and commitment to making a difference in the lives of patients
- Exceptional interpersonal skills
- Valid driver's license and vehicle
- Excellent English verbal and written communication skills
- CPR Certification
- Current immunization record, including 2-step TB test
- Vulnerable sector check and clear background check
