Salary


The estimated salary for this position is CAD $36,810 per annum (based on 32 hours of work per week). This rate includes the relevant shift differential premium where applicable.



Job Description


This position requires a Licensed Practical Nurse with experience in providing holistic care to clients across the primary care continuum. You will be working collaboratively across multiple sites, clinics, and services, or on outreach/home-based visits within an integrated health service delivery model.


You will care for clients and families who are experiencing health challenges, seeking direction/guidance from a Registered Nurse (RN), Registered Psychiatric Nurse (RPN), Nurse Practitioner (NP), or Physician (MD) when needed.


Your responsibilities will include:



  • Assessing, planning, implementing, evaluating, and documenting client care as part of the team.
  • Working closely with RNs/RPNs/NPs/MDs in direct care activities using a team-based care approach.
  • Establishing and maintaining therapeutic relationships based on respect.
  • Supporting clients and their families to achieve and maintain optimal health.


Required Skills and Qualifications


To be successful in this role, you must have:



  • A recognized program completion for Practical Nurses and current practicing registration as a Licensed Practical Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
  • One year of recent related experience or completion of a full final clinical placement in a community setting.
  • Valid BC Driver's License preferred.
