Vancouver Coastal Health is seeking a skilled Social Worker to join our Youth Outreach team in Vancouver, BC. As a Social Worker with our organization, you will work collaboratively with an interdisciplinary team to provide comprehensive social and psycho-social assessments, counseling services, and support to clients with complex mental health and addiction needs.
Key Responsibilities- Conduct thorough social and psycho-social assessments to identify client needs and develop effective treatment plans.
- Provide individual, family, and group counseling services to support clients in achieving their goals and improving their mental health and well-being.
- Collaborate with clients, families, and healthcare professionals to facilitate transitions through the continuum of services and ensure seamless care.
- Develop and maintain client records in accordance with established standards, policies, and procedures.
- Stay up-to-date with relevant legislation, government policies, and procedures to ensure informed practice and advocacy for clients.
- Master's Degree in Social Work from an accredited institution.
- Three years of experience working in a psychiatric setting, providing treatment services to individuals with complex mental health and addiction needs.
- Valid BC Driver's License.
- Current full registration with the British Columbia College of Social Workers.
