Job Title: Continuing Care Assistant

Closing the Gap Healthcare Group Inc. is seeking a skilled Continuing Care Assistant to join our team. As a Continuing Care Assistant, you will provide high-quality patient care and support to individuals in their homes and communities.

Work Setting

Our team works in a fast-paced environment, providing care and support to patients in their homes and communities. As a Continuing Care Assistant, you will work independently and as part of a team to provide patient care and support.

Responsibilities
  • Provide patient care and support, including bathing, dressing, and grooming
  • Assist patients with daily living activities, such as meal preparation and light housekeeping
  • Monitor patients' vital signs and report any changes to the healthcare team
  • Administer first aid and provide emergency care as needed
  • Transport patients safely and efficiently
  • Perform light housekeeping and cleaning duties
  • Coordinate patient care and communicate with the healthcare team
Requirements
  • Basic Cardiac Life Support (BCLS) Certificate
  • Continuing Care Assistant Certification
  • First Aid Certificate
  • CPR Certificate
  • Valid driver's license and reliable transportation
  • Ability to work independently and as part of a team
  • Excellent communication and interpersonal skills
  • Ability to lift, bend, and stand for extended periods
